Overview

Tear trough filler addresses the hollow that runs from the inner corner of the eye across the upper cheek, softening shadowing and the tired look it creates. It is one of the most technically demanding areas of the face, and it is not right for everyone. Where pigment, skin laxity or fluid retention is the true cause, we will say so rather than treat.

Benefits

  • /Softer shadowing beneath the eye
  • /Less hollow, less tired appearance
  • /Smoother transition from lower lid to cheek
  • /Result assessed across the whole midface, not the hollow alone

Suitable for

  • /Genuine volume loss in the tear trough
  • /Shadowing that persists in good light
  • /Patients with good skin quality and minimal fluid retention

Clinical Detail

How we approach this treatment.

Assessment

We separate the three causes of a tired under eye: true volume loss in the tear trough and midcheek groove, periorbital pigmentation, and skin laxity or fluid retention. Only the first responds predictably to filler, and in many cases restoring midface support gives a better outcome than treating the hollow directly.

Placement and product

A firm, low water binding gel is placed deep on bone in very small volumes, most often via cannula to reduce vascular risk and bruising. Product is built conservatively across visits rather than filling the hollow in a single session.

Recovery

Bruising is more common here than elsewhere on the face and can last up to two weeks. Mild swelling settles within a week. The result is reviewed at two weeks before any top up is considered.

Frequently asked.

How long does tear trough filler last?

Results typically last 9 to 18 months depending on the product used and your metabolism.

Will tear trough filler remove dark circles?

Only when the darkness is caused by shadowing from a hollow. Pigmentation and thin, crepey skin will not improve with filler, and we will tell you honestly at consultation if that is what we see.

Is tear trough filler safe?

In experienced hands and with correct patient selection it is a well established treatment. It sits close to important vascular structures, so it should only be carried out by an advanced injector working with cannula technique and a clear plan.

Why might I be turned down for tear trough filler?

Significant fluid retention, notable skin laxity or a hollow driven by midface descent rather than true volume loss all make this treatment a poor choice. Supporting the cheek first often gives a better result.
